Medical Exam for I-485

Could anyone please tell me something about I-693? Someone told me I should get it done and send to the INS center, but I have no idea if I should. My case: FP done Mar. 16th. MA

my lawyer gave it to me with other materials. when I did the medicial exam, I handed over to the doctor. after a week, he gave me back a sealed envelope. the signed I-693 was in the envelop.

then you should submit the sealed envelop to INS with all your 485 package.
