You are really confused. All you need is any photo id (could be driver license, passport is not necessary) and immunization records. That's all, no photo.

Some doctors require you to bring a photo! If you call them to make the appointment, they will tell you what you need to bring. Every doctor has different "rules"! My doctor wanted one photo and she said I would have to get all the immunizations before going to see her. Btw: no, you don't need anything from your family doctor besides immunization records.

Having all immunizations at the medical is much cheaper than getting the missing ones there.

But I'm not sure about the photo requirements. In the past 3 photos were needed, this year just one in Germany.
