Marriage Certificate

I\'ve "DODAMANI NAGAPPA RAJ" in the Birth certificate and "D.N. RAJ" in the Marriage Certificate
My wife has too same problem..
Is that going to be problem?
Do you\'ve any affidativ format for this marriage certificate?Can you send me the format?

Raj,

Since your name on the birth certificate is different from that on
your marriage certificate, You need to provide two affidavits (one
from your father and one from your mother) elaborating the initials
\'D\' and \'N\'. It is not a problem once you provide these affidavits
along with your application. I don\'t have a format for the affidavit.
But I believe it should be of the standard format of declaration,
having your parents name, their relationship with you and stating
the elaboration of the initials \'D\' and \'N\'. Same would be the case
for your wife too.
