regular updates are submitted by your employer to DOH, probably every six monhs or yearly.
DO U THINK IF I ASK THEM THEY WILL PROVIDE ME?
 
Sorry, I'm not there yet. But I'm going to approach my employer first to give me a copy of every communication with DOH because I might need them later. I'm hoping that they'll give it to me.

I'm also going to get a copy of the form employer fills out. This should be available from DOH's web site and see if it says it should be confidential.

If employer is sincere, they should not have problem giving copy to you. If they don't I would be concerned and look for some place else after 3 years are up.

You can call DOH and the should be able to tell you if they got report or not. If DOH donot give you a copy, they can at least tell you if it was satisfactory. And you can tell them you need for as proof as employer would not sign for you. This would be the extreme case and I would wait until last report is filed.

I'll let you know what happens when I encounter this.
 
forgive my ignorance, does the employer fill this form each year, when do we need it ( is it at the end of 5 years)
also if i start my own practice after 3 years , do i myself fill this form and send it to DOH
please clarify
 
Thats a question for Hadron. He hasnt posted in recent days, kind of makes me nervous that we have lost a great resource.
 
It depends upon each State. Normally it is stated in the waiver application package.

I guess you would need it at the end of 3 years if it is LC based GC.

I would get the waiver application package and read through that. And then call the waiver co-ordinator at DOH and ask them. I found most of them very good and open. Of course, I never talked to more popular states like NY, NY, CT etc. I never considered applying in these states.
