Lost previous (MS) OPT Card, applying for (PhD) OPT, Problem?

Hi,

I am about to apply for my OPT for my PhD and the Form I-765,
requires a photo copy of my previous i.e. Master's OPT card.
Unfortunately I am unable to find my previous OPT card as
it is more than 4 years ago that I had got it.

My school does not have a copy of it either. Since I immediately
joined my PhD, I actually did not work on that OPT so I cannot request
a copy from any employer either.
Any suggestions?

I do have the letter with the Lin number dated March 2003 and
the letter of acceptance that I received along with the OPT card
dated July 2003, will it be sufficient if I send a copy of these along
with my OPT application?

Anyone had his/her OPT rejected for this reason or accepted in this
condition?

I am in a similar position. I had a pre-completion OPT for three months, 4 years ago, in the first year of my Ph.D. program. I have no idea where that card is!!

Did you find out more if this is going to be a problem?

So what happened? Did you get your new EAD cards and what did you send instead of the copies. I am also in a similar situation.
