Lost I-94, need you help

Dear friend,

I lost my I-94 (H-1 Visa) and it is a very important document in my GC process. If any one has such experience please tell me how to do? Write a letter to INS and ask resend me I-94 (I know the number) or…..?

I appreciate your help.

I had lost my I94 accidentally 3 years back. I went to local INS. Due to a long queue, I called texas INS, they told me to fill out a form I do not remember the number now ( my employer provided). I think $75 fee and I had a photo copy of my I94.If you have a photo copy processing will be fast.INS texas said it will take 3 months. I got my I94 within 2 months.
