Lost I-94 and Applying for Green Card

Hi,

I came on H1B with Company A. I transferred to Company B. Now I am applying for Green Card through Company B. I have lost my I-94 but I do have xerox of it.

Do I need to apply for new I-94 ??

If I go out of country say Canada, will I need to get new Visa for Company B to get new I-94?

My H1B was filed from India and visa was also issued in India, so do I need to go to india to get my new Visa?
