Lost green card - file I-90 questions

Hi,

Recently I lost my green card in Chicago Airport. After I check the information from USCIS website, I need to file I-90 form.

In I-90 form, I have 2 questions:
1.Consulate where Immigrant Visa was issued or USCIS office where status was Adjusted

I got the notice letter with Green Card from USCIS which was issued by USCIS California Service Center when I got my green card for the first time.

Is USCIS California Service Center the Consulate where Immigrant Visa?

2.Date of Admission as an immigrant or Adjustment of Status

Does mean the date which I received Green Card for the first time?

1. What DO did you have interview at? Put that office on I-90.
2. The date on GC that says something like "Resident since: xx/xx/xx" (i.e. date your permanent residency was approved)
