little bit confused on Medical Report

read thru the form, found out some interesting issues, something just jump into my mind. Here is what I am going to do,
1. go to my regluar doc, get TB shot/update my immunization report. GET PROOF from the doc. will that have to be sealed for the interview?
2. print out I-601 and I-693 from USCIS website, and TB shot/my immunization report, bring all of them to the doc who will do my I-485 medical exame, and let him fill up 601 and 693. Get result and sealed into the envolop for the interview.

that's it right? anything wrong on my procesure? anything more I need to do? does my wife(USC) need to do all those?

Your USC wife has to do nothing.

Yes it is best to get the immunization and TB clearance done and go to the USICS certified surgeon. He will have the forms and will ask you to fill in your info. Once you do that he will take your blood test and will do a PPD or PPT test for TB. BE sure to show him your TB clearance so he doesn’t do the TB test on you.

then within 1-2 weeks he will give you a sealed envelope and that’s your medical report you need to send without opening it.

Best , call around and see what the USICS surgeons are looking for so you can go and get the whole thing by them only. Test plus TB plus immunization , they sometimes have a package deal.

Why do you want to do all this? The civil surgeon takes care of everything you need for the I-693. If you are trying to save money by doing this, you are only increasing the hassles you have to go through to get the medical done. Shop around and you would get prices from $150 to $1500. You may have to drive a bit to get the cheapest one, but its always better to have one doctor deal with the I-693.

You only need I-601 if you are applying for waiver of excludability. Normally you are not required to file this form
 
First stop thinking what you need or what you don't need, the surgeon tells you what you need. Call the surgeon and normally ask what you need before get there, some of them even give you the advice that you can go to your normal health care provider and take some stuff done (s/he will tell you what) and take the results with you when you go to the surgeon. The surgeon follows a manual and paperwork that is given by the CDC, they know pretty well what it should be done.
After all that, just ak for a copy of your paperwork for your records.
