3) Check of $100 in payment of the required I-765 filing fee.
4) Check of $25 in payment of the required Finger Printing fee.
5) Form I-485, Application for Adjustment of Status.
6) Form I-131, Application for Travel Document (Advanced Parole).
7) Attachment of Form I-131: A letter states Reasons for Temporary Travel.
8) Form I-765, Application for Employment Authorization.
9) Form G-325A (four colored copies), Biographic Information of mine.
10) Form I-693, Medical Examination results sealed by Dr. XXX
11) I-765 Signature Card (optional?).
12) Form I-797, Approval notice of I-140 Immigrant Petition
