Letter of Need

I can't seem to find any information online regarding what I need to get the letter of need and from where. Is it ok to go to the local Ministry of Health office in Karnataka state? Also what documents do I need to take to get the signature? Also do they make a fuss regarding the Letter of Need?

It is not clear from your posting as to what you need..are you planning to apply for J1 waiver and talking about NORI?? Please clarify.
