LCA and H-1B six years expiring

I have heard varying stories on this. My employer has filed RIR for my GC processing at the state in March 2002, the six year limit on my H-1B will expire in May 2005. Where do I need to stand at that point in order to apply for an extension of H-1B. My understanding was that I need to have either I-140 or I-485 filed, or is it that the RIR should be more than an year old.
 
With the way things are going these days, I am not sure I will even have the LC approved from the SWA. Does the one year limit on the LC application apply irrespective of where the application is pending at, or does it have to be in the regional office for over a year. Thanks for your reply.
