Lawyer said USCIS screwing up GC issuance...true?

My I-485 case was just approved and on May 6th (Fri) my online status said they were "ordering me a new card". Today, my status changed again to say that "my case was approved and they had mailed an approval notice".

I just spoke to my lawyer and they said recently USCIS has been screwing up this process across the country. People have been receiving approval notices but never got the green card. They were told the GC was in the mail but never got it and had to go to the local USCIS office to get an I-551 stamp and request the GC.

Has this happened to anybody on here? I've read a lot and haven't heard of this happening on this forum...seems like everyone who made it far enough to get the GC, actually got it in the mail.

It can happen. Ofcourse my case of gc is 5 yrs old but I never got it the first time they said they mailed it. I had to pay $120 or $180 (dont remember ) to get a new one. It finally arrived 2.5 yrs after I got my first I-551 stamp.
 
Got my 2 year conditional green card in the mail today. 3 days after I got the update they had mailed it. Very impressive.
