Last name change for Spouse

Hi Folks:

Would you provide some information in changing the last name for spouse at local (Chicago) indian consulate.

I went through their website and found that have very limited information and the phone service was never answered

I'm here requesting information about necessary documents they need and your experience with last name change for spouse and will appreciate your inputs

I changed my wife's last name to my last name at NYC Indian consulate. We went in person and submitted the following docs

-Application is made in the “Application for an Indian Passport”.
-Three photographs
-prescribed fee have to be submitted ($20.00)
-Original Passport
-Notarized copy of marriage certificate
-copy of husband's passport
-Joint affidavit signed by both of you stating the date and time your marriage took place

They told us to call back after 2 weeks. We called them and they said the passport is ready so we went in person and picked it up

also, just notrized copy is sufficient OR original marriage certificate needed ?

here is the format i used...but you can change it according to your needs....its self composed so no hard and fast rules what to write and what not to write..

yes just notarized copy is enough..but if you go in person, its good to carry all the originals just in case if they ask.

I tried to attach a doc but dont know how to ..so i am copying and pasting the contents of the document..please format them.

We had it very easliy done in Chicago. All they required was a copy of the marriage certificate and a copy of the husband's passport in addition to the documents required for passport renewal. We forgot to take a copy of my passport (husband's) and we faxed it to them later.

The folks at the Chicago consulate are very helpful.
