L1 applicant gets Visa, but L2 applicant gets 221(g)

Hi,

I and my wife went for our L1 and L2 Visa interview. The interviewing officer told us your Visa has been approved.

When we went the next day to collect the passports my passport had the L1 Visa stamped but my wife's was given a Yellow paper 221 (G) saying that "Your applicant for Non immigrant visa has been refused under section 221(g) US Immigration and nationality act, pending receipt of additional documentation/information. Once you submit the requested information or the additional processing is complete this office will review your application and submit a final determination. Only the items checked below apply to your case. Please submit the documents requested below along with your passport, this letter and I-797 in the dropbox"

The documents which they have tick marked are "New passport". We are confused why this has been asked as my wife's passport is valid till 2018? Is it because her current passport "might be" blocked because she has had B1-B2 denials on her current passport?

Please help...I'm absolutely perplexed ...PLEASE HELP.
 
L1 visa has to be stamped, or I-797A with new I-94 enough for L2 for my wife and kids

I can not find exact info on this topic.
I am in the US, came on B1 visa and have a I129 petition approved about two weeks ago. Just got the change of status (from B1 to L1A) approval notice in the mail. Is I-797A with my new I-94 are enough for my family to apply for L2 visa back home? Or do I need to leave US, go back home and get L1A stamped before my family can get L2 visas?
Please help!
Of course, I would rather skip a trip, if possible.
