Finally got my GC approval today. Details are below:

RD: 17-Jun-2002
FP1: 08-Aug-2002
FP2: 23-Jun-2004 (Oringal FP date was 03-Aug-2004)
No RFE's

Changed job in Jul'04. Did not inform INS or use AC21. Got approval for me and my wife at the same time today.
