J1R to H1 for Physicians- Need help

I am a researcher on a J1 R visa (Physician from India)

On J1 R i was not allowed to see patients and was mentioned on the IAP66 no direct patient contact

I just received my waiver on basis of No Objection

My Boss is willing to sponsor a H1 visa

I want to start Patient Care if possible and also continue my research

Was wondering that when i fill my JOB description can i put Research, Teaching and Patient care

Does changing myself from only research to clinical patient care and research hamper my possibilities/chances of getting an H1 visa...

h1 is being done by the University
