J1 visa extension

I am currently doing Internal Medicine residency on J1 visa. I intent to do anesthesia residency after that. ECFMG state that I am not eligible to do another residency on my visa.
Please advice.

Sorry, that is unfortunately correct. On a J1, you can change your specialty only once, and that is after a maximum of 2 years in the first. I assume you did 3 years of IM, so you are past that point.

Unfortunately, you won't be able to switch to an H1b visa either. The only option would be an O1 for the anesthesia residency. But recently it is getting apparently very hard to obtain O1's for physicians (even academic folks).

Are you already in your PGY3 ? If you are still a PGY 2, you could switch to the end of the year. (you have to write a brief cover letter to ECFMG if I remember correctly).
