J1 research to clinical

I really need your advice. I am a fulbrighter on a research J1 visa for MPH degree.I just wanna to know if I got the ECFMG. Can I have chance to change my J1 to clinical ?
If not ,Can I take with me the new DS2019 issued to me by the ECFMG and get outside the USA to canada for example to apply for a new J1 for the residency program within the 2 yr HRR.?
also Is it true that fulbrighters have a little chance in getting a transfere of their sponsor to the ECFMG as they are sponsored by Department of State.
I have a lot of questions as I have another option o go to UK which I think has an easier immigration rules for overseas.
Please your kind answers

What I have heard is that you have no chance for a 'no objection' waiver as DOS specifically objects to people staying after their J1 period. I don't know whether a transfer of sponsorship e.g. to ECFMG is a problem for you guys. After all, it is the DOS waiver review division who has to make the recommendation for a waiver, so they are probably within their rights to not recommend a waiver if that would keep you from fulfilling the objective of the program (international exchange).

As for going to the UK: Unless the UK was your country of last permanent residency or citizenship, going to the UK won't help you in serving out your HRR.

I just want to ask if my fulbright will affect my tranfere from research to clinical J1 visa.I do not want to stay here I just want to get the american board after my MPH then I will go out the USA to my country,So what concerns me is can I change it to Clinical type,please your advice
