if a j1 physician marries an LPR or a US citizen, does it affect his or her yearly j1 extensions.what r the chances of rejection if the j1 applies for n extension while in US n what are the chances of rejection if the j1 decideds to go back home on a visit n get the visa stamped from the american consulate in his or her home country

Marriage does not necessarily imply immigration intent, so you should be fine. I did it several times (once in Toronto) after getting married to a U.S. citizen. They never hassled me about it. In Toronto they just asked if I understod that I was still subject to the 2-year HRR. I said "yes" and they did not ask me any other questions.

Of course, if you plan on getting a waiver, then do so after the last extension.

are you also from India.can I get j1 extension in toronto even if I am an indian citizen.and if they do give me an extension and stamp my visa ,can I visit my parents in India and reenter US on that toronto stamped visa.

Pay attention: my screen name is running_swede. Anyway, to answer your other question: yes I think you can go to Toronto if you are from India. I know some friends from China who did it. Of course, China is not India, but I do not see why it should matter that much. You have to make an appointment online. I do not remember the Web site, but I am sure you can find it with a Google search. Plan ahead, because appointments could fill up fast. Also, remember that there is always a small risk in going to Canada. If they for some reason deny your visa renewal, then you would have tp return to India before going back to the U.S. You should be fine if they accept your appointment, but I just want to make sure you know.
