J1, green card and practicing medicine relationship

I have a theoretical question, and would really appreciate an answer since i am quite confused.
Lets assume a j1 holder gets married to a green card holder or a citizen
Now I understand you are required to return to your country for two years;
I also understand you can obtain a green card under marriage but..
Am i allowed to stay in US with green card but not practice medicine, lets say open a business, or am i required to return under any condition. I know i can enter us with tourist visa in that 2 year term but the time i stay in US would not be deducted from my 2 years.
Also what happens if I return back for two years and come back while being married to green card holder. Can i find a job wherever i want since I am a green card holder.

You have to return or get a waiver, married, divine intervention or presidential pardon nonwithstanding. A processing step in every I485 involves a check whether you have fulfilled your HRR, similarly, you can't enter on a an immigrant visa until your HRR is taken care of (in your scenario, you won't get a marriage based GC until after the end of your HRR, so 'staying in the US on your GC' would not be possible.)

While beeing abroad, you can apply for your marriage based GC through consular processing and return after 2 years as permanent resident. As PR, you are free to take any job you want.
