J-1 visa to H1B

I need help, I'm an international medical graduate and I'm planning to go back to my country and do the 2 year home requirement as soon as I finish my residency. I'm planning to do a fellowship after my 2 year home requirement and my question is if i can get a H1 visa before i start my fellowship (I'll do my fellowship in a university medical center).
