I just received my RFI today, same thing multiple times, three yellow letters.When it rains it pours - nothing in years and then three in one day.
They ask for I-693, Supplement to I-693 & G-325A. Initialy I've complied with I-693/I-693 Supplement, but I'll do it anyway again and keep you posted which vaccines I've received. :) :) :)

Anyone from New Jersey who has received recently. I am still waiting. My case status says they have mailed on June 24th.

Hope we will get it tomorrow. I had Thursday's appointment for medical, but I cancelled since I didn't receive RFI so now I have it for Tuesday. Hope we receive shortly.

Hi, Aniket19,

actually you don't have to reschedule your medical appointment. I had it done on Tuesday already, will pick up the result on Friday.
the doctors only need I693 form ( Most of them have the original copy), your A-number and your photo ID. They will provide you a sealed envelop( do not open it ) which you should send to INS with your other required information. Maybe you can finish the medical thing in advance, mail all the stuffs out as soon as you got the RFI........

Hi Aniket, I'm from NJ and recently received the RFI. I moved to PA last month.
 
i didn't get mine to yet--NYC

I didn't get my RFI yet--i live in New York City-- My online profile updated on June 24th and they send me Mail on 24th--but i didn't get the mail yet. Any one have Idea whats going on?

Probably you did not see my previous post. My attorney said that there are time gaps and some of his clients haven't received it although in the email they say "we mailed out on June 24th." My attorney's firm has a number P.O. boxes around the nation. It seems that the mail storm is sweeping from the west coast to east.

Don't worry, 14 days from June 24th, if you still don't get that then, give them a call.

But you already know the content the letter.

So, schedule medical if you haven't done so.
type up the bio. I recommend download the fill-in pdf form and type in. Once you finish the first page, the other pages will be automatically filled out. Sign it. Be consistent with your first bio (the one with I-485). VERY IMPORTANT. My attorney told me if I say in 1999 I lived in CA, in first bio I say I lived in NY in 1999. Then, something is wrong. No guess or estimate on the dates. Especially for refugees and asylees, are moving constantly.

Just some tips on that.

Good tips Windywd, just a correction. G-325 can be filled out electronically. In this RFI, they are asking for G-325A which cannot be filled out electronically. It has to be done manually.

I concur, looked at 325 fillable form which is missing last two pages of 325-A. It was kind of tedious filling process but I got it done.

It took me 3 sets of G325-A, before I could comlete one set without any mistake. You are a very courageous individual, I give it you.

Well I am too late for today's appointment. Anyway, can you tell me what kind of information and what kind of forms I have to fill out.

I know I 693 Do I have to fill 4 copies when I go to see doctor, because when I printed out through internet I got 4 forms.
Also G 325A has 4 copies.
Do they asking for any other forms in RFI which is onthe way?

No, You'll only be filling out G-325A by yourself. Your Doctor will fill out I-693 and I-693A (Vaccination record) for you.
