Is the PR card scanned upon exit?

I want to find out about the following

1. When someone leaves canada, is the GC scanned, i.e do they keep a record of who is leaving and when?

2. I got my PR card giving a friends address in Ottawa where as I work in USA, now I need to appear for a medical exam in Canada, if they indeed scan the card (at the border when PR holder leaves canada) , what would be the appropriate answer to their question 'we do not see that you ever left canada'? I did tell them however that I work in USA and cannot move to canada rightaway.

3. I got my GC for canada in September 2006 and was supposed to get a refund of the excess fee amount, still have not got it, anyone else in similar situation?

i.e do they keep a record of who is leaving and when?

-- These days the airlines keep record and the information may be shared with the immigration authorities.

-- They do not ask these questions because if you did everything truthfully in the best of your abilities, you are not supposed to know why there is no record - and they know this fact.
