Is it OF169 or DS2001?

I have to send the AC140 with I824 receipt to the New Delhi consulate. Apart from the AC140,I797 and the I824 receipt I need to send the OF169 Form. Is it still OF169 or is it DS2001?I have seen both the forms and they look similar, but which one is to be sent?

I don\'t think it matters - they are the same thing. How long did it take for your I-824 receipt to arrive? I waiting for it to arrive and will send packet 3 to mumbai consulate.
