When I went to medical examination, the doctor didn’t ask me immunizations and sealed I-693 for me. Is immunization required?

For my medical/i-693, I took with me my immunization record (translated version). The civil surgeon looked at it, and he noted that my tetanus shot was not up to date. pralay said:Immunization section must be filled up in I-693. It can be filled up by either doing immunization or based on previous immunization records. If that section is blank then you will get RFE in future. There are circumstances where people don't do immunization and leave that section blank - for example pregnancy or any illess where immunization can have a side effect.

By the way, where is doing immunization or based on previous immunization records be filled? What was written on your I-693 copy?
