Is immunization required for I-693?

When I went to medical examination, the doctor didn’t ask me immunizations and sealed I-693 for me. Is immunization required?

Immunization section must be filled up in I-693. It can be filled up by either doing immunization or based on previous immunization records. If that section is blank then you will get RFE in future. There are circumstances where people don't do immunization and leave that section blank - for example pregnancy or any illess where immunization can have a side effect.

For my medical/i-693, I took with me my immunization record (translated version). The civil surgeon looked at it, and he noted that my tetanus shot was not up to date. And so I got the tetanus shot on the spot and that was it.
 
never the less, he should have given you a copy of all the records he "sealed" in the envelope ....that is routine. If he did...did he leave the immunizations section as blank??? If he did not give you a copy, you can request a copy now from his office (civil surgeon's office) as they always keep a copy in your record....and check for yourself

There are only two boxes under Immunization Determination: 1. Applicant is current for recommended age-specific immunization; 2. Applicant is not current for recommended age-specific immunizations and I have encouraged that appropriate immunization be obtained. Which one indicate your immunization was done?

By the way, where is doing immunization or based on previous immunization records be filled? What was written on your I-693 copy?

The immunization record should be within three years and can be completed in either the civil surgeon office or county health department.
 
Hi,

I don't have immunization record and doctor has put in the report that I am not current with immunization which is submitted while filing and later after followup recommended few shots to be taken do we wait till we get a note from INS regarding this?
