IS DS-2001 mandatory form to be filled in packet 3 ???

I am expecting my lawyer to receive packet 3 by end of
this week as it was generated this monday.

I filled only DS-230 I & II and gave it to my lawyer.
He didn't ask me to fill DS-2001 form at all.

Is DS-2001 form mandatory in packet 3 ????
Is it *MUST* component to be sent back to NVC with packet 3 ????

From what I read on the forum it appears that this
form is not mandatory. Several candidates have reported that
they have sent their packet 3s without this form.

In my own case however, my attorney made me fill this form.

I filled out the Packet III forms this month. The only form that was required was DS230-1.
The DS-2001 is no longer sent out in packet 3 nor is it required to be submitted.
