Is a separate affidavit (I-134) required for each person?

My parents have an appointment for visitors visa in Chennai in October (going together). The I-134 form says a separate affidavit is required for each person. But item 3 of affidavit treats as if this is a combined affidavit. It takes info about spouse and children accompanying the person and item 4 & 5 refer to all persons listed in item 3 saying that you are supporting all of them. Then do we still need separate affidavit for my mother and father???
Any insight in this greatly appreciated. Urgent please...

yes, a seperate affidavit for each person. Qs 3 just establishes how many people are accompanying the person. In fact the instructions state that a seperate affidavit has to filed.
