IRS9003 form- optional or mandatory?

$$$$$ In my interview on Jan 22 it was asked though as per my lawyer it was not needed$$$$

It is part of the list the consulate asks for

And as Patta has mentioned it is very easy to get so why not take it ?

IRS 9003 & Documents:

Most people tend to play safe by gathering as much supporting documentation as they can for the consular interview. As mentioned before, IRS 9003 is a standard document on the list of "Packet 4" docs.

The form IRS 9003 by itself is fairly easy to prepare (IRS claims you need only 5 mins), and asks you for basic information about our tax (US Federal) filing history for last three years. To "play safe", you should keep:

W2s & related docs (e.g., 1099) for the last three years
Copies of tax returns
Tax transcript provided by IRS - you can obtain these by calling IRS

It is just one yellow sheet...

Friends, It is just an yellow sheet which my attorney got it for me. It mentions when was the last tax returns filed in the US.
In addition, I have got previous 3 yrs tax transcripts from the local IRS. It was just 2 minutes to get them.
Is there anything more for IRS9003 documents?

IRS9003 is that Yellow form.. but you can print a downloadable copy on white

and it is readily acceptable

Tax Transcripts for last 3 years make your IRS related docs complete.
Some people also carry W2. In this case More is Good... More Docs you have it gives you more confidence....

do we need to get IRS signatures/seal or notarized or ..

It is just our own statement and has no validity check from an authority?

I am primary applicant & Spouse is leavinig for India, I do not have the IRS 9003 form.
