As part of the long list of papers the INS asked me to bring medical exams + immunization forms (I-693) at the interview, unless submitted. We submitted the forms 1 year ago, however I am afraid this will not be the end of the story :-(
In the section \'Results\' of the immunization form there are multiple choices, including
- applicant may be eligible for blanket waivers
- request waiver for religious reasons
- history complete
- does not meet requirements
All the immunization forms we got 1 year ago have the first option selected. In one of the cases the doctor pointed out that more immunizations are needed (Td). On the other immunization forms the first option is also selected but we did not receive further instructions (2 different doctors).
I went back to the doctor, did another immunization and got a new form with \'history complete\' this time. However it is a lot more complicated to do the same thing for the other forms - long story.
Could you guys check your forms and let me know what the results section is set to in your case? I am interested specially by people who already went through the interview (come on guys, help please).
In case we do need to get an updated immunization form, do we need to go to the same doctor or can we use a different one?
