Interim EAD from San Jose, INS office

I just crossed the 90 days after filling my I485 and I765. Could someone tell me, what are the documents that I need to carry to get an interim EAD. My case(I-765) has not been approved yet, so i guess there shouldn\'t be any problem in getting an interim EAD. Do i need to pay $100 fee at the counter?

Pls post your experience in getting interim EAD from San Jose, CA.
