Interim EAD at Newark, need info

Could some one please tell me,

Have any of you got an interim ead from newark office, and if so, what is the procedure, do I have to take an appointment.

My I765 rd was Sept. 18, 2003, and has been above 90 days, still have not received EAD.

Also if you have a telephone no. for Newark office for appointments, would really appreciate it.
