Intention to deny or RFE? Difference?

Does anybody know what is the difference between "intention to deny" or "RFE"?

I check the web, it is saying something like:
... we mailed a notice requesting additional evidence or information in this case. The notice explains in detail what additional material is needed...

How many days should the reply be sent back to USCIS? 30 days or 12 weeks for this kind of notice?

That sounds like a typical RFE. You will be given 84 days to reply.

Doubt

Dear my friends, thanks a lot!!! Do you know what will be a general rate for hiring a lawyer to help this, or it depends on how complicated case is? Do we have to resubmit most of items, like rewriting a CL and submitting recommendation letters both new and old ones if the RFE just ask to prove the case of meeting all 3 prongs set by NIW...?
