Intent to Deny - Reason

Hi
      My wife\'s I485 got Intent to deny eventhough she was in H4 all along and I got approved 2 weeks back.Today my lawyer got notice it states to prove my wife was in H4 status from 95 to 97.She has H4 visa stamp from 95-97 on passport and lawyer has said he will take care of it.
Will Keep fingers crossed.
