Instructions for DS-230

Hello,

I couldn't find any instruction for filling out DS-230 form in the forums. If you know any would you give me a link?

My qustion is:

How will I fill item #30? I've been in United States for 8 months, so I will specify this information in this section? Will I write the state (Ohio) in "province" section?

Yes that's right, you have mention that in details... how many months you spent, which state and province, and the purpose of the visit. as well as you will bring the documents in the interview to prove that "like the visa in your passport and the ticket if you still having it.

Thank you. I have another question: What should I write for "place of birth"? I'm from Turkey and should I need to write the city province also. Or, is just TURKEY enough for place of birth?

well, what if you were born in USA? you just can't write USA and that's it!! you will be have to mention which State !...
So, as for Turkey, You have to mention which city you were born in, Istanbul, Izmir etc...

Yes, but on my birth certificate my "place of birth" is seen as KOLAY which is a village of a city. So what will I write. And izmir istanbul etc are province not the city or town

In my case small to large: KOLAY / SULUOVA / AMASYA / TURKEY which is VILLAGE / TOWN-CITY / PROVINCE / COUNTRY

what's my place of birth in the form now?

Use the dob stated on your birth cert.
 
on my birth cert. my dob is stated as the name of the village only? is it a good idea to write down the name of the village only? without the province and country?

Contact the US consulate in your country. They should be familiar with this matter and will be able to advise you.

This is what I wrote on my form and nothing was raised during my interview:
Name of village, Country
 
Regarding question 31b, Person(s) named in 14 and 29 who will follow you at a later date.

We're not sure whether or not to list me (the daughter). It's my mom who's filling out the form, my dad would follow her straight away, I, however, am over 21. Yet I have the right to apply for a visa as soon as my parents receive their visa.
My dad and I both say that I shouldn't be listed as a person who will follow at a later date because it's not even sure I will be following yet and I'm not a current visa applicant.
My mom says I should be listed since I plan to follow as soon as I can apply for a visa.
My dad and I think this question just concerns those who currently are applying and simply won't be going straight away for whatever reason.
