I decided to pay the $120.00 to be on the safe side.
As for the documents required, they are all listed on the INS website www.ins.gov under forms. Go to I-765 and you will get about 9 pages of instructions and a one page form. I sent the following:
Check for $120.00 per application
2 photographs with my name and A number pencilled on back
Top portion of mailer from original EAD card
Copy of I-140 approval notice
Copy of I-485 receipt notice
Copy of I-94 front and back
Copy of EAD card front and back
DNA sample (kidding)
If you can read English and follow instructions you don't need a lawyer.
