Information on FBI Fingerprinting card

I got my fingerprinting done at a local agency on the FBI fingerprinting card. The person who took the fingerprints just signed and dated the card. He did not put any stamp or anything else to identify himself or the agency. Here are my questions / Concerns:
1. Is any such information is required?
2. If yes, what and where should he put it?
3. What other information do I need to put in the card?
4. My wife does not have SSN as she is on H4. Is it required?
