Information needed regarding I-485 application

Dear Friends,

June visa bulletin will be out anytime soon. I have applied for my I-140 on Dec. 5, 2005 (which is also my PD) in the EB1 (extraordinary ability) category (India). Hopefully, if the trend of PD continues in the current visa bulletin, my PD may become current so that I can apply for I-485 on June 1, 2006. I have few questions before I submit my I-485 (I have not filed through a lawyer).

1. Do I have to prepare a separate application form for my wife?
2. Does my wife also have to do the medical check up?
3. Do I need to send any other application form along with I-485 (besides EAD and AP)?

Don't understand that. You can file only I-485 for you and all family members, EAD and Ap are not mandatory.
