I would like know as I, my wife and my sons I-485 has been approved 12/21/2004, my question is, Do I need to take an Infopass appointment separately for each individual or together?

Yes, u need to take seperate appointment for each one.
I would suggest u to use different computers to schedule an appointment as sometimes that website doesnt allow more than 1 appointment at a time.
