INFO PASS appointment and required documents

Gurus,

Can someone pls give me some info about the documents needed for the passport stamping.
Do i need to take appointment seperately for my wife or just one appointment is enough ? Do they need passport photos for passport stamping ?

Documents you will need:
1) Approval notices
2) Passports
3) Passport style Photos - 2 each
4) All EADs/ APs, current I-94s if any, and any other approvals you may have (H1/H4)

You will need to schedule two appointments, but both can go together.
 
I got my I485 approval from NSC and just recently moved to California. If I schedule info pass appointment in California and show my ILLOINS driver's license, would it be okay or would it cause some problem?
