Indian birth certificate getting issued, but my name has changed few years back

Hello - I do not currently have a birth certificate. I am going to request for a birth certificate to be issued, however since my name has changed (the new name is in the passport & visa) should I request the current/new name be entered in the birth certificate or should I request my name at birth be used for the birth certificate? I have all the proof for the name change.

School certificates - Has the name at birth
New/Current Passport - Has the NEW name
US Visa - Has the NEW name
All US documents - Has the NEW name
Birth Certificate - Requesting to be issued [What name should be in the birth cert since it is getting issued only now]

Your birth certificate can and should be issued only under your birth name. Also obtain affidavits from close relatives regarding your birth and change of name since a late issued birth certificate alone is sometimes not acceptable for purpose of US immigration. I presume you are filing I-485 for yourself.

Thank you nkm-oct23 for the response. I will then request the birth certificate under my birth name. I have the documentation from the local municipality and official documents related to the name change. So with the birth certificate (with birth name), affidavit from parents and name change documents, can I file for 485? Will there be anything else I would need related to the birth certificate? Really appreciate your help.

I think you should be able to file I-485 with the documents you listed. Please remember to submit only copies of documents along with the application, don't send any originals.

If name change of due to marriage, marriage certificate should satisfy, if it is voluntary change then court document is required. In G-325A you would mention your current legal name and where it says all other names you would mention birth name and or all other names previously used.
