Incorrect Information in Medical Form

Guys;

I recieved Meical Form from CIC for my wife, there are couple of mistakes in the Section A of the form

1) First name boxe has her name and the my name, my name is
spelled incorrectly.

2 )Date of birth is in correct.


What should I do in this situation?

I don't think that should create any problem. May be you can call and/fax Buffalo for clarification to make sure.
