In Preparation for Packet 3

All,
I have sent the fee bill to NVC and the next thing is to wait for Packet 3.
In preparation for the same. is there a check list of things that we need to keep ready before the packet 3 arrives to the lawyer office ?
Also if there is a link for the application form that comes in packet 3 we can download and fill it up just for informational purposes for us and the lawyer
pls provide pointers for both
