I have worked at 1 other state (CA-paid only CA tax for 2 years in 2007 and 2008) in last 5 years but have maintained my residence (as I have a house) in NC and don't recall if I filed AR11 all the time. Now I'm back to NC and planning to file my N400 from here. Shall I put the address of CA and NC for the last 5 years or only NC is fine since I always maintained a house here.

From Uscis.gov,
"You do not need to include temporary addresses as long as you maintain your present address as your permanent residence and continue to receive mail there.

When sending us a change of address, you do not need to include numerous last addresses; only the most recent last address is needed."

So just send in the form for AR-11 for your NC address asap. Obviously its late, but better late than never. It should not affect your citizenship interview, so long as the AR-11 matches your N-400.

Good luck with your app.
