Yes, you very much can. Just get those records on some official letter with name and address of the issuing authority/person on it.
A letter from any nursing home or any doctor should work.
Worked for me. But your civil surgeon could be more fussy, so better ask them about what they need.
Also you don't have to submit this proof to immigration deptt, you need to just take that to the civil surgeon and they prepare the required form for you.
