The form I-683 requires the following vaccines/tests:
syphillis, HIV, Tuberculosis, MMR
However, there is another supplement form that lists a lot of other vaccines such as polio, DPT, influenza etc. Do we need to get vaccines for all that?

You don\'t have to get all the shots since you have passed certain age.
You will have to take the following:

1. MMR
2. Tetanus
3. TB Test
4. HIV Test

This is sufficient for your age.
