Immunization is incomplete

I found this information: "Immunizations are not required, but the applicant should be encouraged to go to a physician or local health department for appropriate immunizations" (Medical Clearance Requirements for Aliens Seeking Adjustment of Status; Form I-693).

My wife refused a tetanus shot (she needs only this vaccine). She is a nursing mom and we plan a baby and she afraids that vaccines can harm babies.
Can we have any problems if she will not receive this vaccine (processing will be delay etc.)?
Is it any risk that a doctor will not give her needed documents?
 
Tricky situation, I would recommend consulting to a doctor who is not related to your immigration case, to see whether the shot would have any negative effect to your baby. If not, you will probably have to get it done, since the doctor will not sign the document unless all shots are okay.
