Immigration forms

Please can someone give me some advice.I am busy getting all my forms ready to send them of, but have two questions.

1. Do we send all the forms to the same place ? (Chicago)
2. On the I-130 form .....Line 22 ......says:

your relative is in the us .............at the USCIS office in:

what office name do you put in there ????

I hope someone can help me !!!

ok thank you very much. So that means i have to put in Charlotte as it is the closest office to me , but i dont my forms there.
