I693 supplemental form

Lawyer send me 485 documents long back in Dec 2005 (when I-140 was approved) with I693 supplemental form. I went to USCIS website and don't see this form there.

Do we need to submit it along with I693 ?

Since my lawyer is out of office for next 10 days will not be able to check with him the same.
