I130 consular BMB?

i paid IV processing fee for my daughter, i am greencard resident. i have to submit DS230. her consular will be BMB.
can you help me what kind of document i have to submit to NVC with document cover sheet?
SHOULD I HAVE TO SUBMIT FOLLOWING DOCUMENTS TO NVC?
1)DS 230 part I(sign) and II(without sign)
2)Birth certi original and translation one
3)police report
4)copy of passport bio page
